The 30-year-old passed away within hours of the operation, with his "sudden" death believed to be the first case of its kind.
The patient had visited a private plastic surgery clinic in Stockholm, Sweden, to undergo both a penile elongation and penile enlargement.
During the surgery, he was put under general anesthesia before the elongation procedure was carried out successfully, a report states.
He then underwent the enlargement, which saw fat cells extracted from his lower abdomen and injected into the shaft of his penis.
Before the cells were transplanted into his genitals, they were washed with saline and antibiotic solution, according to the document.
However, towards the end of the surgery, at around 4pm, the patient's heart rate suddenly rose and his oxygen levels dropped.
He also began suffering from hypotension - low blood pressure, according to the case report in the Journal of Forensic Sciences.
He was incubated and given medication to treat his symptoms, but within half an hour he had gone into cardiac arrest.
Medics performed CPR on the man as he was transported to hospital, but their attempts to revive him were sadly unsucessful.
He was pronounced dead shortly before 6pm, the report says.
The man, who had no medical history except for "mild asthma", was later found to have died from a pulmonary fat embolism.
It was found that fat loosened during the operation travelled in "punctured veins" to his lungs, which were left "congested".
